Familial hemiplegic migraine (FHM) falls within the category of migraine with aura. In migraine with aura (including FHM) the neurologic symptoms of aura are unequivocally localizable to the cerebral cortex or brain stem and include visual disturbance (most common), sensory loss (e.g., numbness or paresthesias of the face or an extremity), and dysphasia (difficulty with speech).

These symptoms are classified as brainstem aura because they are believed to be produced by dysfunction of the regions of the base of the brain called the brainstem. In most cases, the migraine aura may progress from visual symptoms to sensory symptoms, to language and motor symptoms — in some sequence over the course of several minutes. In

Many patients who have attacks with brainstem aura also report other attacks with typical aura and should be coded for both 1.2.1 Migraine with typical aura and 1.2.2 Migraine with brainstem aura. Many of the symptoms listed under criterion C may occur with anxiety and hyperventilation, and therefore are subject to misinterpretation.

Migraine with brainstem aura symptoms. The most common symptoms that people have during migraine with brainstem aura are: 1-3. Dizziness or vertigo (feeling like the room is moving) Slurred speech (also called dysarthria) Ringing in the ears, trouble hearing, or double vision (also called diplopia) Feeling unsteady or uncoordinated.
